Digital hospital passport survey

The North East and Cumbria Learning Disability Network are currently developing a digital learning disability hospital passport.

The hospital passport is a document owned by an individual, that they can bring to support their care if accessing secondary care settings where staff may not know the person or their needs.

To support the development of a digital hospital passport we ask staff who work in primary and secondary care to take 5 minutes to answer the short survey.
